Frailty & Failure to Thrive
Frailty is a diagnosis, not a personality trait. Recognizing it early — and treating what's reversible — is one of the highest-yield things primary care can do in the home.
How we care for it at home
- Comprehensive geriatric assessment: strength, gait, nutrition, cognition, mood.
- Medication review to remove the ones that make frailty worse.
- Nutrition and protein strategies that fit real appetites.
- PT referrals and safe strength work at home.
What we watch for
- Unintended weight loss and sarcopenia.
- Slowed gait and grip strength over time.
- Recurrent infections, falls, and hospitalizations.
- Caregiver capacity and social support.
