Frailty & Failure to Thrive

Frailty is a diagnosis, not a personality trait. Recognizing it early — and treating what's reversible — is one of the highest-yield things primary care can do in the home.

How we care for it at home

  • Comprehensive geriatric assessment: strength, gait, nutrition, cognition, mood.
  • Medication review to remove the ones that make frailty worse.
  • Nutrition and protein strategies that fit real appetites.
  • PT referrals and safe strength work at home.

What we watch for

  • Unintended weight loss and sarcopenia.
  • Slowed gait and grip strength over time.
  • Recurrent infections, falls, and hospitalizations.
  • Caregiver capacity and social support.
