A clear, human path from your first phone call to a steady care plan at home. No hoops, no surprises — just the steps we take with every family.

You reach out
Call us or send a message through the contact form. Share a little about the patient — their situation, who is helping them, and what's prompting the call. There's no obligation, and the first conversation is free.

We gather basic information
A member of our team follows up to collect the essentials: current diagnoses, medications, recent hospitalizations, insurance, and where the patient lives. We'll ask about the family's goals so the first visit is genuinely useful.

We confirm you're a good fit
Not every patient is right for a home-based practice, and not every practice is right for every patient. We take a quick look at clinical need, service area, and insurance to make sure we can actually help. If we can't, we'll point you somewhere that can.

First visit — in person or telemedicine
In most cases a physician comes to the home for an unhurried evaluation: full history, exam, medication review, and time to talk with family. When appropriate, we can start with a telemedicine visit and follow with a house call.

A care plan you understand
After the first visit we put together a written plan: what we're treating, what to watch for, what to do when something changes, and who to call. Nothing is left implicit.

Ongoing visits and support
We schedule follow-up visits at a cadence that fits the situation — weekly, monthly, or as needed — with phone and message access between visits. As things change, the plan changes with them.
