For many chronic conditions, what changes first is a number: a weight, a blood pressure, an oxygen level. When appropriate, we coordinate connected devices in the home so those changes are caught days — sometimes weeks — before a crisis.

What we monitor

  • Blood pressure, for hypertension and heart failure.
  • Weight, for fluid changes in heart and kidney disease.
  • Pulse oximetry, for COPD and other lung conditions.
  • Blood glucose, for insulin-treated diabetes.

How it fits into care

  • When indicated, devices are simple — one button, cellular-connected, no Wi-Fi setup.
  • Readings are reviewed by your physician or a trusted monitoring partner, not lost in a call center.
  • Concerning trends trigger a phone call the same day.
  • The data is integrated into your regular primary-care visits, not treated as a separate program.
