  • What to write into a POLST or living will so it actually gets followed.
  • How to talk about all of this with adult children, spouses, and grandchildren.

How the visit runs

  • A physician at your kitchen table, not a clipboard at a clinic.
  • Family invited — in person or by phone.
  • Documents completed, signed, and shared with your other providers.
  • A follow-up visit if new questions surface later. They usually do.
