Wound Care

Pressure injuries, venous ulcers, and diabetic foot wounds don't heal from a clinic. They heal from repeated, careful attention where the patient actually lives.

How we care for it at home

  • In-home wound assessment, staging, and dressing changes coordinated with nursing.
  • Offloading, compression, and positioning that fit the actual home.
  • Nutrition and glycemic control aimed at healing.
  • Coordination with wound care specialists and vascular when needed.

What we watch for

  • Signs of infection, osteomyelitis, and cellulitis.
  • Circulation and neuropathy that predict non-healing.
  • New pressure points on chairs, beds, and shoes.
  • Pain, which is under-treated in chronic wounds.
